Identification & Insurance

Please bring a valid photo ID and your current insurance card to your appointment. 

Medical Records & Recent Labs

If you have recent lab results, imaging, or medical records related to your visit, bring copies or have them sent ahead of time. 

Forms, Medications & Questions

Complete forms ahead of time, bring a medication list, and jot down any questions you may have.
