Fever Management for Infants

Fever in babies is one of the most common reasons parents worry and call the doctor. A fever is usually a sign that your child’s immune system is doing its job, but in young infants it can be more serious.

What Counts as a Fever?

For infants, a fever is defined as a rectal temperature of 100.4°F (38°C) or higher.

Rectal temperatures are the most accurate in babies. Ear and forehead thermometers can be less reliable in very young infants.

For babies younger than 3 months:

Any rectal temperature of 100.4°F (38°C) or higher is an emergency call to your pediatrician.

Do not give fever-reducing medicine (like acetaminophen) unless your baby’s doctor specifically instructs you.

In this age group, fever can be the only early sign of a serious infection, which is why doctors take it so seriously.

How to Check Your Baby’s Temp.

  • Use a digital thermometer.

  • For infants, a rectal temperature is preferred.

  • Follow the instructions on the thermometer and clean it well after use.

Helping Your Baby Feel Better:

You can:

  • Offer plenty of breast milk or formula

  • Dress your baby in light clothing

  • Keep the room comfortably cool

  • Offer extra cuddles and comfort

When Fever Is Less Concerning

For older infants and toddlers, most fevers are caused by viral infections and improve in a few days. Many viral fevers sit between 101–104°F and are not harmful by themselves as long as your child is drinking and urinating normally.

Focus less on the number and more on how your child looks and acts.

When to Call the Doctor

Call immediately if your infant:

  • Is younger than 3 months with a fever of 100.4°F or higher

  • Is having trouble breathing

  • Is very sleepy, hard to wake, or not acting normally

  • Shows signs of dehydration

  • Has a stiff neck or persistent vomiting

  • Has a seizure
